Peabody have an exciting opportunity for a full time Health and Housing Coordinator. As an integral member of the discharge team at Darent Valley Hospital you will provide an effective, timely and robust approach to patient discharge.  In this fast-paced environment you will find, investigate and provide resolutions to issues preventing a patient from going home safely.  You will at times be required to provide discharge support to associated cottage hospitals, step down bed facilities and other acute hospitals in Kent. The role will also require you to undertake home visits as necessary, actively assessing the safety and suitability of the home environment.

This role is seen as an integral part of the Integrated Discharge Team and is responsible for avoidance of hospital admissions wherever possible along with providing a timely proactive discharge planning service to ensure:

  • Support planning with best possible outcomes for the patient.
  • Timely access to range of community based health, social care services and voluntary sector services.
  • Optimum use of acute/community and social services resources.

As the successful Health and Housing Coordinator you must:

  • Be able to drive and have access to a car.
  • Have proven knowledge, experience and understanding the needs of vulnerable people, either in a voluntary or professional role.
  • Have demonstrable experience in achieving designated outcomes for customers.
  • Be motivated and able to work using own initiative and with minimum direct support.

You will be working as part of the Health and Housing team based in various hospitals in Kent. This job role is based at Darent Valley Hospital and is being offered initially on a fixed term contract until 31st March 2022 in line with funding available.
